Harc, Inc.’s Community Companion Home (CCH) Program invites families from diverse cultures and backgrounds to open their homes to a person with an intellectual or developmental disability. By becoming a part of the CCH Program, you provide a safe, nurturing environment where individuals with disabilities receive personalized support, attention, and the opportunity to lead a positive and fulfilling lifestyle. Participants are encouraged to engage in family activities and community outings, helping them to work towards their personal goals.

Each participant benefits from a tailored individualized support plan that reflects their aspirations, challenges, strengths, and skills. Harc’s CCH Coordinators conduct monthly check-ins to ensure ongoing support and care.

Families are carefully matched with a Harc participant based on the specific needs of both the family and the individual. To ensure the highest standards of care and safety, each family is licensed and undergoes a thorough background check.

Long-Term Stability and Care

CCH placements are designed for long-term care, offering individuals the security of remaining in the same home for as long as needed. This model promotes continuity of care, fostering trust and deep connections between the individual and the CCH provider. Many families have maintained care for 20, 30, or even 40 years, with some homes passing the caregiving role to the next generation to ensure the individual continues to receive familiar, compassionate care.

A Personalized, Family-Like Environment

Individuals living in a CCH enjoy a family-oriented atmosphere where they are fully integrated into the provider’s daily life. This creates a nurturing environment that often leads to strong, lasting relationships. Many individuals remain with their CCH provider for decades, with bonds that can span generations, ensuring a stable and familiar home environment.

As a CCH family, you will have access to Harc’s 24-hour on call system, respite, and recreational activities. All of which support a well-rounded life for the individual and the CCH family.

To become a CCH you must follow the steps below:  

  1. Receive Introduction Packet
  2. Application Submission (3 references)
  3. Background Check
  4. Harc Provided Training
  5. Interview and Home Study
  6. Match Search
  7. Meet and Greet
  8. Transition (match trial)
  9. Match Agreement
  10. Continuing Contact

Supports Offered by Community Companion Home Providers

CCH providers are responsible for ensuring the individual’s well-being across various life areas:

Daily Life and Recreation

Encouraging participation in day programs, community engagement, and recreational opportunities that promote independence and social inclusion.

Life Skills Development

Supporting individuals in developing essential life skills to foster independence and personal growth.

Financial Oversight

Assisting with financial management, completing redeterminations, and ensuring eligibility for essential services such as Social Security and Medicaid.

Healthcare Coordination

Scheduling and attending medical appointments, managing medications, and coordinating overall healthcare needs.
